SBLEDS SLP Application For Admission
** To Complete The Application Form You Must Have an Internet connection; email account; and the latest version of Adobe Acrobat (.adobe.com).

** This form can only be completed using Internet Explorer.
** You may save the form and submit the application when all required fields are completed and all documents are attached.
** You must attach electronic PDF copies of your, ID, matric certificate, degree/diploma, etc.
** Certified copies of degree/ diploma/ senior certificate s are required. Foreign qualifications must be evaluated by SAQA before being sent to the SBL ( .saqa.co.za).
** Attach the relevant foreign certificate and SAQA evaluation in one PDF file in the appropriate attachment area.
** Please ensure that you submit the application form and the relevant documents before the application closing date.
** A non-refundable application fee of R100 must be paid after receiving a student number.
** Programme fees are payable as set out in the financial guidelines and the application cannot be finalsied unless the required documentation and application fee has been received. For payment details please refer to the financial guidelines
** After completing all the required fields and attaching the required documentation, click on “Submit Application” at the bottom of this form and application will be submitted to the SBL.

Required Attachments
** Please attach the additional documents before submitting the form.
** Failure to submit these documents with your application form will result in your application not being processed
** A non-refundable application fee of R100 must be paid; please provide proof of payment (preferred method of payment is Electronic Funds Transfer (EFT))
** A certified copy of your Identity Document/ Passport (copy of driver’s license is not accepted)
** A certified copy of your matric certificate
** An abridged Curriculum Vitae of yourself (Total number of years’ work or managerial experience;
** Job title (eg Human Resources) Manager; Responsibilities and number of years’ work experience in current position;
** If less than three years work experience with current employer;
** provide past employment history; Your initials and surname;
** Contact particulars of a person at your employer
** Marriage certificate (Marriage certificate must be submitted if surname on qualification or ID differ)
** Sponsorship letter (If you are company sponsored kindly provide a sponsorship letter)
